Privacy collection notice

The personal information you provide on this form is collected by the Queensland Racing Integrity Commission (QRIC) for the purpose of managing your report, including providing feedback on the outcome of your report (if requested). The personal information collected on this form may be used or disclosed by QRIC for the purpose of assessing, investigating, actioning and responding to your report.

In some circumstances QRIC may share your report with another agency in accordance with section 53A or section 261 of the Racing Integrity Act 2016 (Qld). This may occur for example, where a report contains information that will assist another agency in the performance of their functions. QRIC will not otherwise use or disclose your personal information without consent unless required or authorised by law. Your personal information will be handled in accordance with the Information Privacy Act 2009 (Qld).

Anonymity

You may choose to remain anonymous when you submit a report; however, in some instances, it may be difficult to investigate certain aspects of an anonymous report. For example, where further information is required from the complainant in order to investigate further. Please ensure there is sufficient information in the report to enable us to conduct an investigation. If you provide your name and contact details, we can provide you with feedback about your report.
